Mods and damage.

We have just loaded some tweaks to armour and weapon modding:

* Apparently our HELP DAMAGE was hopelessly outdated, leading to a lot of confusion - it should now explain things better

* Restrictions on which mods can be used together have been relaxed, as they were rather pointless anyway; mods of the same type still don't stack
- you could stack a source mod with a type mod before, but it wasn't documented correctly. Now it is!

* Damage reports now indicate the damage source, as well as the damage type

* We'll be monitoring mod effects and usage and making further tweaks as necessary
