Bug fixes and improvements.

Hello again Sector,

We're back for more bug fixes and improvements!

- the issue with pirate refineries blowing up in Unknown locations has been resolved.
- fixed a potential exploit which could award double vouchers from a Cosmpiercer capture.
- the range in space at which Cosmpiercer generators will reset has been increased. This means you can now fly further away from the generators without them resetting. This resolves some lingering issues with generator resets.
- REVERSETHRUST now actually works!
- Fixed an issue with using the Heartbeat Sensor on ships.
- The Melter untrapped error should now be vanquished for all eternity.
- COSMPIERCER POWERS should now properly display in fixed width.
